This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Uses and disclosures for treatment, payment, and operations
We use your protected health information (PHI) to provide and coordinate your care with independently licensed providers and the dispensing pharmacy; to process payment for services and products; and for health care operations such as quality review, training, and business management.
Other permitted disclosures
We may disclose PHI without your authorization when required by law, for public health and safety activities, for health oversight, in judicial or administrative proceedings, to law enforcement as permitted, and to avert a serious threat to health or safety. Any other use or disclosure — including most marketing and any sale of PHI — requires your written authorization, which you may revoke at any time in writing.
Business associates
We contract with vendors (hosting, electronic records, telecommunications, pharmacy, and shipping partners) who may handle PHI on our behalf. Each is bound by a business associate agreement requiring them to safeguard your information.
Your rights
- Inspect and obtain a copy of your record, including an electronic copy.
- Request an amendment to information you believe is incorrect or incomplete.
- Request an accounting of certain disclosures we have made.
- Request restrictions on uses or disclosures, including a restriction on disclosure to a health plan for services you pay for in full out of pocket.
- Request confidential communications at an alternative address or phone number.
- Receive notification of a breach of unsecured PHI.
- Obtain a paper copy of this notice on request.
Complaints
To exercise a right or file a complaint, contact us at info@elevehealthco.com. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
Changes to this notice
We may change this notice and apply the revised terms to all PHI we maintain. The current version is always posted here. See also our Privacy Policy.
