Telehealth Informed Consent
This consent explains how care is delivered through Elevé Health Co. and must be reviewed and accepted before a licensed provider evaluates you. By submitting an intake form you acknowledge that you have read and agree to it.
Nature of telehealth
Telehealth is the delivery of health care using electronic communications rather than an in-person visit. Your evaluation may be conducted asynchronously — meaning a provider reviews the written information, photos, and history you submit and responds without a live visit — or by phone or video where required by your state.
No provider-patient relationship until accepted
Submitting an inquiry, creating an account, or paying for a program does not create a provider-patient relationship and does not guarantee a prescription. A relationship begins only when an independently licensed provider accepts your case. If a provider determines a therapy is not clinically appropriate for you, no prescription will be issued and you will not be charged for medication.
Benefits and risks
Benefits include convenient access to licensed clinicians and home delivery. Risks and limitations include: the provider cannot perform a physical examination; information you supply may be incomplete or inaccurate, which could affect clinical decisions; technology failures or delays may occur; and in rare cases a condition may go undetected that an in-person examination would have identified. You may be asked to obtain laboratory work or an in-person evaluation before or during treatment.
Medications and compounded products
Prescribed medications may be compounded by a 503A US compounding pharmacy. Compounded medications are not FDA-approved and are not reviewed by the FDA for safety, effectiveness, or manufacturing quality. They are prepared for an individual patient pursuant to a valid prescription. Side effects, drug interactions, and allergic reactions are possible; report any adverse reaction to your provider and seek emergency care when appropriate.
Records, privacy, and recording
Your information is stored in an electronic record and shared only with the clinicians, pharmacy, and vendors involved in your care, as described in our Notice of Privacy Practices. No part of your consultation will be recorded without your written permission. No electronic transmission can be guaranteed to be fully secure.
Alternatives and right to withdraw
You may seek in-person care at any time and may withdraw this consent or discontinue telehealth services at any point without affecting your right to future care. Withdrawal does not retroactively reverse services already provided or medications already dispensed.
Emergencies
Elevé Health Co. does not provide emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. For mental health crises, call or text 988.
Eligibility
You must be at least 18 years old and physically located in a state where our providers are licensed at the time of service.
